To become an affiliate of RecruitScreen, please complete the Lead Referral Agreement Form below. As you find qualified leads that are interested in using our services, please complete the Lead Referral Registration Form for each client. You may e-mail, fax or mail these forms to us.

• Lead Referral Terms and Conditions - This doc-ument lists the terms and conditions of the Lead Referral Agreement.

• Lead Referral Agreement Form - This document needs to be completed by the individual that will be referring business (the affiliate).

• Lead Referral Registration Form - This docu-ment is to be submitted to RecruitScreen, LLC with the prospective lead information.
